Same Day Admission
We know the kind of hectic situation it could be after being discharged from a hospital. Surlely after being discharged, patients are quite eager to get situated with their care plan for treatment. This is why we choose to always admit patients as soon as possible. We can always admit a patient on the same day of the referral.
Durable Medical Equipment
Durable medical equipment or DME, is an ancillary service that is utilized when patients need some type of equiptment that the physician thinks would be necessary for the patient to come back to a state of restored health. These are things like hospital beds, bedside commodes, shower benches, medical supplies, etc. Our agency has contacts with durable medical equipment companys willing to work with our patients doctors to get the best possible care that can be afforded.
Translation Services
We have spanish speaking nurses that work very hard at ascertaining everything about our spanish speaking patients. We want to let you know language is not a barrier for us. We even have a company that will translate any language we need them to via telephone.
Pre-Discharge Assessments
Upon request we will arrange for a nurse to come out to the facility that the patient is being discharged from, and we will perform a review of the patient clinical records and an assessment of the patient at the facility before the patient is sent home. Our registered nurse will then consult with the patients physician to identify the patients needs and the needed coordination of care.
Our Service Guaranteed
Upon admission we give patients a patient satisfaction questionaire to complete after services have begun. Alimed Home Health Care wants to make sure the patients in our service are happy with their treatment by our staff. This is why we conduct supervisory visits to the patients home to assess if goals in the patients plan of care are properly being met.
